Hospice Registered Nurse - full time

Home Health & Hospice Merrimack, NH 03054, USA Description Mon - Fri, 12:00pm - 8:00pm or 11:00am - 7:00pm. 40 hours / week. Registered Nurse (RN) full time opening in our hospice division for patients residing in their own home or in facilities/nursing homes we serve. The primary function will be admissions, with additional visits as needed. We are growing and you can too! Responsibilities Completes an initial home health assessment of patient and family to determine hospice care needs; provides a complete physical assessment and history of current and previous illness(es). Utilizes the nursing process in planning, providing and evaluating skilled needs. Completes a comprehensive patient assessment. Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions, including the patient and family on the planning and revising process. Qualifications – Required Graduate of an accredited school of nursing. Registered as RN with New Hampshire’s Board of Nursing. A minimum of one year recent acute care experience. Ability to exercise initiative and independent judgment. CPR Certification. Satisfactory criminal and motor vehicle records. Once fully trained, minimum requirement is one weekend a month with pay $4.00 an hour additional, and 1 winter and 1 summer agency holiday required pay time and a half. Qualifications – Preferred Bachelor degree in nursing. Community Health/Education experience. Certification in Hospice and Palliative Care, Oncology or end-of-life experience (preferred, willing to train). EPIC experience preferred, willing to train. EMR software proficiency.
