divh2Job Title/h2pResponsibilities:/pulliCoordinate the Minimum Data Set (MDS) process for residents in accordance with regulatory guidelines/liliConduct comprehensive assessments of residents health status and care needs/liliCollaborate with interdisciplinary teams to develop individualized care plans/liliEnsure accuracy and completeness of MDS assessments and documentation/liliStay updated on changes in regulations and requirements related to MDS/liliTrain and educate staff on MDS processes and documentation standards/liliParticipate in quality improvement initiatives and audits/liliCommunicate effectively with residents, families, and healthcare professionals/li/ulpRequirements:/pulliCurrent licensure as a Registered Nurse (RN)/liliPrevious experience as an MDS Coordinator preferred./liliKnowledge of MDS process and regulations/liliStrong attention to detail and organizational skills/liliExcellent communication and interpersonal abilities/liliAbility to work collaboratively in a team environment/liliProficiency in electronic health record systems preferred/li/ulpEqual Opportunity Employer/p/div
