Ideal Candidate:

The Case Manager has a deep passion for non-profit work and feels aligned with the faith-based mission of Several Sources. This position is responsible for assisting the Spanish speaking clients in the Special Families, sidewalk advocates referrals and Lifeline programs by providing resources and connecting them to Several Sources Shelters services, checking in, and following up on goals and objectives for the client. Advocate for external services and government benefits for clients as needed.

Duties and Responsibilities:

Case Management

• Provides direct case management services and support to mothers and babies, managing referrals and resolving issues. Provides guidance in helping clients to identify needs.
• Assist clients in completing government assistance applications (food, medical insurance, childcare, housing subsidies).
• Collaborates with key partners, providing financial literacy, housing, substance abuse treatment, employment, childcare providers, and criminal justice agencies, in accordance with Several Sources Shelters policies, procedures, guidelines and applicable state regulations.
• Facilitates the Multi-Disciplinary Team (MDT) team meeting and members’ work. Receives case summaries to integrate them into one weekly summary report.
• Participate in a team approach to treatment including coordinating client services with other Several Sources Shelters staff, the MDT and outside provider agencies.
• Assists in securing housing, gainful employment, education, driver’s license, ELC classes and other referrals to support service plans.
• Encourages client’s active participation in case management services. Ensures case management is strength based.
• Conduct assessments of children’s needs as well and provide appropriate referrals.
• Develop, implements, reviews, revises service plans under supervision of the CEO.
• Monitors client outcomes on as determined based in individual. weekly basis.
• Participate in limited home visits and/or visits with clients and safe sites to provide material and supplies or accompany to services.

Documentation

• Meets with CEO weekly and provide written monthly reports as instructed.
• Initiates timely documentation of services delivered through eKYROS ensuring confidentially.
• Prepares and maintains client charts, records, and reports.
• Reviews charts to ensure all necessary documentation to receive assistance is complete.

Community Relations

• Maintains the Several Sources Shelter’s resource and referral database and keeps it updated monthly.
• Develop relations with other providers and referral agencies.
• Understands, researches, and analyzes community and population’s needs.
• Uses analysis to provide goal recommendations.
• Advocates for clients at the program level and documents the need for exceptions or changes in policies and procedures on a case-by-case basis.

SKILLS/KNOWLEDGE REQUIRED:

· MSW degree in human services field, LCSW preferred.

· A minimum of two years’ work experience in non-profit agency operations, specifically in advocacy, case management, or Social Work.

· Bilingual in Spanish oral and written (interpretation proficient). Knowledge of Spanish translation.

· Knowledge of the mission, objectives, policies, programs, and procedures and of the principles and practices of non-profit organizations preferred.

· Ability to facilitate the MDT team communications, prepare minutes, ensure follow-through with the client/MDT team members regarding appointments, and make sure there is cooperation between both parties.

· Ability to maintain collaboration with the team of service providers within Several Sources Shelters.

· Ability to organize and prioritize projects and time management.

· Strong oral and written communication skills.

· Proficiency in Microsoft Office, including Word, Excel, Outlook, and PowerPoint.

PHYSICAL REQUIREMENTS/WORK ENVIRONMENT:

High energy level, comfortable performing multi-faceted tasks in conjunction with day-to-day activities. Ability to lift at least twenty-five pounds.
