Care Coordinator PositionWe are seeking an organized, proactive, and compassionate Care Coordinator to join our growing team. This key role is responsible for coordinating client services, staffing cases with qualified caregivers, maintaining clear communication with clients and families, and supporting the recruitment and onboarding of caregiving staff. The ideal candidate thrives in a fast-paced environment, communicates effectively, and has experience in home care coordination or healthcare staffing.Client Communication & Care CoordinationServe as the primary point of contact for clients and their familiesConduct intake calls and assist with service setupCoordinate care schedules based on client needs and service plansMaintain regular follow-up to ensure client satisfactionAddress concerns and resolve scheduling or service issues promptlyStaffing & SchedulingMatch caregivers to clients based on skills, availability, and compatibilityManage and update caregiver schedules dailyEnsure all shifts are covered and proactively address call-outsMaintain accurate scheduling records and documentationCaregiver Recruitment & HiringAssist with recruiting, interviewing, and onboarding caregiversVerify credentials, certifications, and required documentationEnsure compliance with state licensing and agency policiesSupport caregiver engagement and retention effortsAdministrative DutiesMaintain accurate client and employee filesEnsure documentation meets regulatory standardsCollaborate with management to improve processes and client servicesQualifications:Experience in home health care, healthcare staffing, or care coordination preferredKnowledge of licensed home care agency operations and compliance requirementsStrong organizational and multitasking skillsExcellent communication and interpersonal skillsAbility to work independently and as part of a teamProficiency with scheduling software and Microsoft OfficeProblem-solving mindset with strong attention to detailPreferred Skills:Experience working in a Licensed Home Care Services Agency (LHCSA)Understanding of caregiver credentialing requirementsAbility to handle high-volume scheduling in a fast-paced environmentWhat We Offer:Competitive salary (based on experience)Supportive and team-oriented work environmentOpportunities for growth and advancementMeaningful work that impacts lives every day
