- Shift: Days, Monday - Friday, 8-hour shifts
- Hours per Week: 40
- Estimated Weekly Pay: $2,260 - $2,430
- Active Occupational Therapy (OT) license (state-specific licensure required for Everett, WA assignment)
- Minimum of 1 year of recent Home Health OT experience required (home health occupational therapist experience)
- Proficiency with HCHB (Homecare Homebase) documentation system
- Ability to travel within a 50+ mile radius of the Everett office
- Strong organizational, time management, and patient-centered care skills
- Provide high-quality occupational therapy services to patients in their homes (home health OT care, functional rehabilitation)
- Complete clinical assessments, treatment plans, and therapeutic interventions focused on activities of daily living (ADLs) and functional independence in the home environment
- Travel to patient locations within a 50+ mile radius as needed for care coordination
- Meet productivity expectations (minimum 30 points per week) while maintaining clinical quality
- Complete all required documentation by 10:00 AM the following day using HCHB (accurate clinical documentation)
- Collaborate with interdisciplinary team members-nurses, PT, SLP, physicians-to ensure optimal patient outcomes
- Educate patients and families on therapy plans, home programs, equipment use, and discharge recommendations
