Hospice RN Case ManagerLocation: Marlborough, MAEmployment Type: Full-Time (Flexible scheduling may be available)On-Call: Shared after-hours on-call rotation as appropriate*We are seeking a self-lead, highly organized and accountable RN who can manager a fluctuating caseload. If you value meaningful relationships, collaboration, autonomy, and making a genuine difference every day, we would love to meet you.Position SummaryThe Hospice RN Case Manager serves as the clinical leader for an assigned patient caseload, coordinating and managing all aspects of hospice care. This role requires clinical excellence, strong organizational skills, sound judgment, and a caring heart.
