Role Description The Community Liaison is a full-time, hybrid role based in The Woodlands, TX, with some work-from-home flexibility. This role focuses on building and maintaining relationships with referral sources, community organizations, and healthcare partners to increase awareness of Enhabit’s home health and hospice services. Day-to-day responsibilities include conducting community outreach, attending local events and meetings, and presenting information about services to patients, families, and professionals. The Community Liaison collaborates closely with clinical and operational teams to ensure seamless coordination of care and a positive experience for all stakeholders. This role also involves monitoring market trends, identifying opportunities for engagement, and providing timely, responsive customer service to community partners. Qualifications Candidates should possess strong Community Engagement and Community Outreach skills to build and sustain relationships with local organizations and referral sources. Candidates should possess excellent Communication and Customer Service skills to effectively represent Enhabit and support patients, families, and partners. Candidates should possess knowledge of Home Care and an understanding of home health and hospice services to accurately convey service offerings. Relevant experience in healthcare, social services, marketing, or community relations is beneficial. Ability to work in a hybrid environment, manage time effectively, and travel locally within The Woodlands, TX and surrounding areas as needed. Strong organizational, presentation, and relationship-building abilities; proficiency with standard office software and digital communication tools. High school diploma or equivalent required; an associate or bachelor’s degree in a related field (such as healthcare, communications, marketing, or social work) is preferred. #J-18808-Ljbffr
