About the Role:

The Home Health Community Liaison serves as a vital connection between home health care providers and the community, ensuring that patients receive seamless access to necessary health services. Minimum Qualifications:

• Minimum of 2 years experience in healthcare, community outreach, or patient advocacy roles.
• Strong knowledge of home health care services, healthcare systems, and community resources.
• Excellent communication and interpersonal skills to effectively engage diverse stakeholders.
• Valid driver’s license and reliable transportation for community visits.

Preferred Qualifications:

• Bachelor’s degree in healthcare administration, social work, nursing, or related discipline.
• Experience working in home health care or case management.

Responsibilities:
