Senior Social Worker (FGP) - Manhattan, Medicine-Cardio, Part Time

Position Summary Senior Social Worker (FGP) – Manhattan, Medicine-Cardio, Part Time. Provides psychosocial assessment and counseling, including discharge planning, under the supervision of a Social Work Supervisor or Director. Job Responsibilities Provide timely psychosocial assessment and evaluations, using clinical knowledge and interviewing techniques to gather social history, define problems, and develop treatment plans. Assist teaching of psychosocial issues related to health and illness, community resources, and social work services. Ensure patients receive services in accordance with departmental policies through regular chart reviews. Represent the Department of Social Work in hospital and citywide committees and task forces. Provide task supervision and organize psychodynamic support groups for patients and families. Serve as a resource for staff and other hospital employees; organize training for new or transferring staff. Participate in interdisciplinary planning and unit‑based meetings; contribute to program and policy development. Supervise interns and conduct orientation patient/family groups on diagnosis, treatment, and post‑hospital care. Prepare statistical documentation and present psychosocial information in interdisciplinary rounds and conferences. Care for patients of all ages and participate in individual/group supervision. Mobilize community agencies and support networks for patients and families. Coordinate with Care Managers for effective throughput and timely discharge planning. Minimum Qualifications Minimum: Master’s degree in social work (CSWE accredited) or equivalent; New York State licensure as a Licensed Clinical Social Worker (LCSW) or pending examination; 4+ years of post‑MSW health‑care experience. Preferred Qualifications Previous experience in a hospital or health‑related organization. Key Competencies Effective communication with all levels of the organization.
