Key Responsibilities\nIn your role as an LCSW, you will engage in a variety of critical tasks aimed at improving the mental health and overall experience of our patients. Below are some of the essential responsibilities you will undertake:

- Conduct thorough clinical assessments and develop individualized treatment plans that align with the unique needs of patients facing terminal illness and their families. \n
- Utilize evidence-based therapeutic interventions, including Cognitive Behavioral Therapy (CBT) and mindfulness techniques, to provide emotional support and enhance coping strategies. \n
- Maintain meticulous clinical records to comply with HIPAA regulations, ensuring patient confidentiality is always upheld. \n
- Collaborate seamlessly with an interdisciplinary team, which includes physicians, nurses, and spiritual care providers, to ensure an integrated approach to patient care. \n
- Participate in regular case reviews, sharing insights and strategies to continually enhance the quality of care provided to patients. \n
- Provide psychoeducation to patients and family members, equipping them with essential resources and knowledge to navigate the complexities of grief and mental health challenges associated with end-of-life care. \n
- Stay informed on current research and best practices within the field of hospice care, integrating new findings into your therapeutic approach. \n
Your primary focus will be on patients undergoing terminal illness within a hospice environment. You will provide compassionate support to individuals facing not only medical challenges but also emotional and existential concerns related to their condition. Working closely with families, you will assist them in coping with grief, loss, and the impending transition, ensuring that they feel supported throughout their journey.
Qualifications\n- \n
- A Master’s degree in Social Work from an accredited program. \n
- Current licensure as a Licensed Clinical Social Worker (LCSW) in New York State. \n
- A minimum of three years of clinical experience in mental health therapy, preferably within a hospice or palliative care setting. \n
- Proficient understanding of solution-based practices and interventions tailored to the needs of individuals facing end-of-life issues. \n
- Outstanding communication skills that facilitate building trusting therapeutic relationships with patients and their families. \n
- Strong problem-solving skills to effectively address complex emotional and psychological issues. \n
