Lead CNA Job DescriptionThe Lead CNA plays a critical leadership role in delivering high-quality, client-centered home care services. This position combines hands-on caregiving, staff training, and quality assurance oversight to ensure services meet regulatory standards and align with Thrive's holistic care model.$20-21 an hourResponsibilitiesDirect Client Care & CoverageProvide hands-on care to clients.Cover open shifts as needed to ensure continuity of care.Travel throughout service areas to support client needs.Ensure client safety and maintain a safe home environment.Report any changes in the client's condition promptly.Quality Assurance & ComplianceDevelop, implement, and oversee quality control processesConduct caregiver performance evaluations and field assessmentsPerform care plan audits to ensure compliance with regulationsEnsure adherence to company policies and home care standardsAssist with incident reporting and corrective actionsTraining & Staff DevelopmentTrain and onboard Personal Care Coaches (PCCs)Provide ongoing coaching and mentorship to caregiversEducate staff on: Best practices, Care standards, Regulatory requirementsSupport continuous professional developmentCare Plan OversightReview and evaluate individualized care plansCollaborate with nurses and administrative staff to update care plansEnsure services align with clients' unique needs and preferencesClient Satisfaction & Relationship ManagementMonitor client satisfaction and service qualityAddress concerns and resolve issues proactivelyBuild strong relationships with clients and familiesFoster a positive care environmentDocumentation & ReportingMaintain accurate and timely documentation of: Client care, Caregiver performance, Quality assurance activitiesEnsure all records are compliant and audit-readyAdministrative & Team SupportAssist with scheduling assessments and onboarding processesSupport administrative tasks and team initiativesParticipate in interdisciplinary team meetingsCollaborate across departments to improve service deliveryContinuous ImprovementIdentify gaps in care delivery and recommend solutionsImplement initiatives to enhance quality and efficiencySupport innovation in home care practicesSkills/QualificationsBachelor's degree in healthcare administration, nursing, or a related field OR a mix of education and experience.Certified Nursing Assistant certification (required).CPR and First Aid certification (required).Must have a valid Driver's License.Previous experience in a healthcare or home care setting.Knowledge of home care regulations and industry best practices.Excellent communication and interpersonal skills.Attention to detail and a commitment to continuous improvement.Familiarity with relevant technology and software for documentation and reporting.This position does require travel throughout our service regions.Technical RequirementsMust own a smartphone with data.Must own a personal computer to use for work purposes.Must own a personal vehicle with insurance that meets minimum state guidelines.Must be willing to travel to all our service regions.Must be willing to report to the office as needed.Florida Background Screening Notice Pursuant to House Bill 531 (2025) and effective January 1, 2026, Florida requires employers to provide applicants with access to the Florida Background Screening Clearinghouse.
