Community Primary Care Provider-Nurse PractitionerFull Time and Part TimeNow Conducting Visits in Queens and Nassau Counties!Parker At Your Door, a medical house calls program serving the greater New York Metro region, is seeking a Primary Care Provider Nurse Practitioner to provide quality primary medical care to our patients in the comfort of their homes. Transportation to home visits in the community is provided by an EMT DriverPosition OverviewAs a Primary Care Provider, you will be responsible for assessing members with multiple comorbidities residing in the community.
