Schedule: Full-Time | Monday-Friday, 8:30 AM-4:30 PM

In this community-based role, you'll help individuals receiving behavioral health services access essential resources, overcome challenges, and work toward their personal recovery goals. What You'll Do
- Manage a caseload of up to 30 individuals receiving behavioral health services.
- Develop personalized service plans that promote recovery and independence.
- Connect clients with housing, healthcare, employment, and community resources.
- Monitor client progress and provide crisis intervention when needed.
- Collaborate with families, providers, and community support teams.
- Maintain accurate and timely documentation.
- Participate in a rotating 24/7 on-call schedule.
- Bachelor's degree in Psychology, Social Work, Criminal Justice, Counseling, Education, or a related field OR
- High school diploma with qualifying social science credits and relevant human services experience.
- Valid Pennsylvania driver's license with no more than 3 points.
- Strong communication, organizational, and problem-solving skills.
- Ability to complete required background clearances.
