We welcome clinicians and healthcare professionals from diverse backgrounds who are passionate about making a meaningful difference for patients living with complex chronic conditions.Role OverviewAs a Nurse Practitioner at Client, you’ll be a core member of an interdisciplinary care team, providing comprehensive medical care and chronic disease management to a focused panel of patients living with CKD and other complex conditions. Your work will center on preventive care, chronic disease management, patient education, and care coordination all through a value-based, relationship-centered approach. You’ll work closely with physicians, Social Workers, Care Navigators, specialists, and community resources to ensure care is coordinated, connected, and centered around the whole person.
