Key Responsibilities

As an Internal Medicine Nurse Practitioner, you will carry out a variety of essential tasks, including:

• Conducting thorough initial patient histories and performing comprehensive physical examinations to assess health status.
• Documenting and communicating detailed plans of care, ensuring that all assessments and patient preparations are well-coordinated.
• Performing daily patient rounds to monitor residents' health, addressing any concerns or needs that arise.
• Writing orders for medications, laboratory tests, and diagnostic imaging, ensuring precise and effective treatment plans.
• Interpreting laboratory results and diagnostic tests to guide clinical decision-making.
• Collaborating closely with attending physicians, nursing staff, and other healthcare providers to optimize patient care outcomes.
• Drafting discharge summaries and prescriptions as part of the discharge process, ensuring patients understand their post-discharge care instructions.
• Participating in ongoing educational and research activities designed to improve patient care practices.
• Performing related duties as required, maintaining adherence to all essential functions as defined by the Americans with Disabilities Act.

Patient Population / Client Focus

The residents of our Assisted Living Facility consist of older adults who require a variety of care levels ranging from support with daily living activities to advanced medical management. You will have the opportunity to build meaningful relationships with residents, enabling you to tailor care to their individual needs and contribute to their overall quality of life.

Qualifications

To be considered for this role, candidates must have:

• A Master's Degree in a Nurse Practitioner Program, along with National Board Certification in your area of specialty.
• A current license to practice as a Nurse Practitioner in New York State, along with a valid Registered Professional Nurse license.
• A collaborative practice agreement with designated protocols filed with the NYS Department of Education or a collaborative relationship attestation as outlined in the Nurse Practitioner Modernization Act of 2015.
• Current certification in Basic Life Support (BLS).

Preferred Experience

While not mandatory, preference will be given to candidates with experience in managing elderly populations within an Assisted Living or similar care setting. Familiarity with electronic health record systems and a demonstrated ability to work collaboratively as part of a multidisciplinary team will be advantageous.

Compensation and Benefits
