Admissions Nurse

Job Summary The Admission RN is responsible for the entry of patients and families into Penn Medicine Hospice. As a member of the Interdisciplinary Team (IDT), the Admission RN provides direct care to adult patients and demonstrates safe practice across clinical judgment, nurse-patient/family relationships, clinical leadership, and scholarship. The RN works with the health‑care team to coordinate care across the continuum, reviews potential client documentation, arranges initial contact, and communicates plans to patients, families, and referral sources. The RN provides assessment to assure appropriate care levels, facilitates symptom management, and practices within hospice regulations and reimbursement criteria. Responsibilities Continuous Quality Improvement (CQI): utilize data and QI methods to identify problems and opportunities for safe, timely, efficient, effective, and equitable care. Evidence‑Based Practice and Research: evaluate and integrate the best current evidence with clinical expertise and patient/family preferences for optimal health care and system effectiveness. Leadership: collaborate and apply innovative, systems‑thinking approaches to engage in evidence‑based problem solving and decision‑making that promote effective change within a complex care delivery system. Person and Family‑Centered Care: recognize patients as partners in providing compassionate and coordinated care, based on respect for their preferences, values, and needs. Professionalism: demonstrate a commitment to the nursing profession through lifelong learning, adherence to ANA’s Code of Ethics for Nurses, participation in professional organizations, and community outreach. Safety: minimize risk of harm to patients, families, providers, and self through system effectiveness and individual performance. Technology/Informatics: use appropriate information and technology to communicate, manage knowledge, mitigate error, and support decision‑making across the continuum. Teamwork: engage in cooperation, coordination, and collaboration to provide safe, quality outcomes within inter‑ and intra‑professional teams, including virtual teams.
