Care Management Coordinator

Supporting administrative care management activities, the full-time Care Management Coordinator will perform outreach, answer inbound calls, and schedule services for Medicaid LTSS members aged 21 and older in a remote environment. Key Responsibilities Provides outreach to members via phone to assist with care plan next steps and connect them to health plan and community resources Serves as the front-line support for member and provider inquiries, explaining care plan procedures and protocols Documents and maintains non-clinical member records in accordance with state and regulatory requirements Required Qualifications High School diploma or GED 1-2 years of related experience in a remote call center environment 2+ years of experience processing healthcare or insurance authorizations 2+ years of experience making outbound calls to members or providers Knowledge of Social Determinants of Health (SDOH) needs and local resources
