Home Health Admissions RN: Start-of-Care Specialist

Altruist Home Care Solutions is seeking an Admissions Registered Nurse to perform comprehensive admissions assessments and OASIS documentation for home health patients in the Dallas area. You will coordinate care with physicians, patients, and the interdisciplinary team, initiating plans of care and ensuring safe, timely transitions of care.
The role requires completing start-of-care assessments, medication reconciliation, patient education, and adherence to HIPAA and agency policies.
