This position involves delivering thorough nursing care to patients with cardiac conditions in both outpatient and step-down environments. Key Responsibilities: Conduct patient assessments and monitor telemetry. Administer medications as prescribed. Develop and implement personalized rehabilitation care plans. Educate patients and their families on lifestyle changes and effective disease management. Collaborate with cardiologists and a multidisciplinary team to ensure optimal patient outcomes. Facilitate safe transitions of care for patients. Promote a patient-centered and safety-oriented culture while enhancing your clinical skills. Qualifications: Current Registered Nurse license. Experience in cardiac care or rehabilitation is preferred. Strong communication and interpersonal skills. Ability to work collaboratively within a team.
