Hospice Care Planner Overview
- The Hospice Care Planner role focuses on evaluating patients for hospice or palliative care services, ensuring seamless transitions and compliance with regulations.
- Collaborate with healthcare teams to provide clinical leadership and support for community and facility-based hospice programs.
- Engage in planning, customer service delivery, and service area expansion initiatives to enhance care quality.
- Coordinate internal and external clinical marketing efforts to promote hospice services effectively.
- Conduct community outreach and education to raise awareness about hospice and palliative care options.
- Provide direct clinical support to patients and families, ensuring compassionate and personalized care.
- Work in a dynamic healthcare environment with opportunities for professional growth and development.
- Contribute to the development and implementation of innovative hospice care strategies.
Hospice Care Planner Key Responsibilities & Duties
- Evaluate patients referred for hospice or palliative care services, ensuring eligibility and appropriate care plans.
- Facilitate seamless transitions for patients into hospice or palliative care programs.
- Provide clinical leadership and oversight for hospice program growth and development.
- Coordinate marketing efforts to promote hospice services within the community and healthcare facilities.
- Engage in community outreach and educational activities to inform stakeholders about hospice care benefits.
- Deliver exceptional customer service and support to patients and their families.
- Collaborate with interdisciplinary teams to ensure comprehensive patient care.
- Participate in service area expansion initiatives to meet growing patient needs.
Hospice Care Planner Job Requirements
- Associate of Applied Science (AAS) degree from an accredited institution; BSN preferred.
- Minimum of two years’ experience in community health, hospice, or oncology nursing.
- Strong clinical expertise and ability to evaluate patient needs effectively.
- Sales or business experience is preferred to support marketing efforts.
- Excellent communication and interpersonal skills to engage with patients and healthcare teams.
- Ability to manage and oversee community and facility-based hospice programs.
- Knowledge of applicable laws, regulations, and agency policies related to hospice care.
- Motivated and sharp professional with a focus on delivering high-quality care.
