Contract (13 Weeks) | Full-Time | Days | Monday – Friday | Onsite / Field-Based

Key Responsibilities

• Provide speech-language pathology services to patients in a home health setting
• Evaluate and assess patients' speech, language, cognitive-communication, and swallowing disorders
• Develop and implement individualized treatment plans based on patient needs and goals
• Monitor and document patient progress, adjusting treatment plans as appropriate
• Collaborate with interdisciplinary care team members to coordinate patient care
• Educate patients and caregivers on therapeutic techniques and home exercise programs

Required Qualifications

• Master's degree or higher in Speech-Language Pathology
• Current and valid state licensure as a Speech-Language Pathologist in California
• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from ASHA
• Experience providing speech-language pathology services in a clinical or home health environment
• Valid driver's license and reliable transportation for home visits

Preferred Experience / Skills

• Prior experience in a home health setting
• Familiarity with home health documentation and electronic medical record systems
• Strong interpersonal and communication skills for working with patients and families in a home environment
