Why Choose This Opportunity Lead and develop a compassionate home health clinical team Help shape a collaborative, accountable, and patient-centered culture Clinical Manager Responsibilities Overseeing patient care, clinical staffing, referrals, and assignments Ensuring timely starts of care and seamless care coordination Reviewing plans of care, physician orders, and clinical documentation Conducting OASIS reviews to support accuracy, quality, and compliance Monitoring patient needs and ensuring care plans remain appropriate Coaching clinicians and providing ongoing education and feedback Supporting recruiting, interviewing, onboarding, and staff development Partnering with physicians, patients, families, providers, and vendors Monitoring key clinical and operational performance metrics Providing field support when needed Qualifications Unrestricted Massachusetts RN License 2+ years of RN experience in Home Health or Hospice Supervisory or clinical leadership experience preferred Strong OASIS, care coordination, and clinical knowledge Ability to learn systems such as Homecare Homebase and Workday Willingness to travel within branch’s coverage area $95,000-$120,000 annually Full-time Keywords: Home Health Registered Nurse, Leadership, Patient Centered Care, Skilled Nursing, Care Coordination, Medication Management, Wound Care, OASIS Assessments, Care Plans, Patient Education #J-18808-Ljbffr
