What You’ll Do As a Nurse Practitioner, you'll provide comprehensive, person‑centered medical care to individuals with intellectual and developmental disabilities and behavioral health needs. Conduct comprehensive health assessments and physical examinations. Diagnose acute and chronic medical conditions. Develop, implement, and manage individualized treatment plans. Prescribe medications and monitor treatment effectiveness. Collaborate with physicians, nurses, therapists, and interdisciplinary care teams. Educate individuals, families, and caregivers on health conditions, medications, and wellness strategies. Maintain accurate, timely clinical documentation using electronic health records. Participate in care planning, quality improvement initiatives, and clinical best practices. Promote safety, dignity, inclusion, and person‑centered care in every interaction. What We’re Looking For We're seeking a clinician who is compassionate, collaborative, and committed to improving the health and well‑being of individuals with diverse needs. Qualifications: National certification (ANCC, AANP, or equivalent). Master’s or Doctoral degree in Nursing from an accredited program. Current BLS/CPR certification. 1 year of experience in primary care, community health, behavioral health, or working with individuals with intellectual and developmental disabilities preferred. Strong assessment, diagnostic, and clinical decision‑making skills. Excellent communication, documentation, and organizational abilities. Experience using electronic health records (EHRs).
