Hybrid Case Manager: Older Adults & Disabilities

East Bay Community Action Program (EBCAP)

East Bay Community Action Program (EBCAP) is looking for a compassionate Case Manager to support older adults and adults with disabilities in Rhode Island. This full-time position involves conducting assessments, developing care plans, and coordinating services to promote independence and well-being. Ideal candidates will have a client-centered approach and two years of case management experience.
