Work Environment

This position is onsite and involves providing care in the community, allowing for a dynamic work environment where no two days are the same. You will work mostly in patients' homes, which requires flexibility, adaptability, and a deep understanding of individual patient needs. Key Responsibilities

• Conduct comprehensive assessments of patients, taking into account their medical history and current physical condition to develop tailored treatment plans.
• Communicate effectively with patients and their families by explaining evaluation results and treatment approaches, engaging them in the goal-setting process to promote adherence and satisfaction.
• Implement and adjust treatment protocols based on evaluation findings, patient progress, and feedback, employing evidence-based practices to enhance patient outcomes.
• Collaborate with a multidisciplinary team to coordinate care, consult when necessary, and document changes in patient conditions to modify treatment plans accordingly.
• Ensure thorough documentation of patient care activities, maintaining accurate records that reflect the patient's journey, treatment progress, and discharge planning.
• Demonstrate time management skills to meet productivity expectations while maintaining a structured schedule, attending meetings punctually, and participating in marketing efforts to promote our services.
• Contribute to achieving departmental goals by complying with all relevant regulations and participating in quality improvement initiatives.
• Pursue professional development and continuing education opportunities to enhance your skills and competence in physical therapy.
• Drive excellent patient experiences by adapting to patients' needs, removing barriers to care, and engaging in initiatives aimed at improving accessibility for all clients.

Patient Population / Client Focus

Your role will focus on a diverse patient population, including individuals recovering from surgeries, managing chronic conditions, or recovering from injuries. Your ability to connect with patients in their homes fosters a comfortable environment for their rehabilitation journey, allowing you to provide personalized care tailored to their unique needs.

Qualifications

• A minimum of a Bachelor's degree in Physical Therapy from an accredited institution.
• A valid physical therapy license as required by state regulations.
• Current certification in Basic Life Support (BLS) for Healthcare Providers as per the American Heart Association.
• Strong interpersonal and communication skills to effectively engage with patients and their families.
• Dedication to providing empathetic care and a commitment to ongoing professional development.

Preferred Experience

Compensation and Benefits
