Evaluations include assessing health status, strengths, care needs, and preferences. The results guide the development of individualized long-term care service plans. Review of consumer medical documentation and health referral forms is relevant to the case. Evaluation data must be entered into electronic evaluation forms and transmitted as required. Any concerns or conflicting information during the evaluation process must be documented. Emphasis on continuity of care is crucial to reduce fragmentation, duplication, and gaps in treatment. Discussing health care options, support needs, and service vendor options is part of the role. Initiate communication between eligible consumers for enrollment into a managed care health plan. Enhance communication and collaborative relationships with interdisciplinary care team members. Collect quality review data and document to support outcome measurements and record case notes. Identify opportunities for health promotion and illness prevention. Maintain a comprehensive knowledge of community resources and network services for the target population. Adhere to all standards established for this position. Perform other duties as assigned by the Regional Nurse Manager or project management.
