Job Title: Registered Nurse (RN) Profession: Nursing Specialty: Home Health Duration: 260 days Shift: 2x3 days Hours per Shift: Not specified Experience: Minimum 2 years in a home health setting within the last 10 years License: Current RN Licensure in Client Certifications: Basic Life Support (BLS) required before start date Must-Have: - Two years of experience in a home health setting within the last ten years, including experience working in the field for a Certified Home Health Agency, conducting assessments similar to OASIS, providing primary care to homebound persons, and case management for persons receiving home and community-based services. - Recent experience conducting comprehensive assessments of patients. Description: Candidates must have the ability to review health status questions and make evaluations based on criteria for enrollment. Candidates should be comfortable working with technology. Knowledge of the principles and practices of general professional nursing is essential. Understanding of laws and regulations governing professional nursing practices is required. Excellent organizational and interpersonal skills are expected. Strong written and verbal communication skills are important. Ability to demonstrate sensitivity to the concerns of others is necessary. Candidates should thrive in a fast-paced, deadline-oriented work environment. Capability to manage multiple complex tasks simultaneously is required. Candidates will perform initial in-home pre-evaluations of patients, including assessments of health status, strengths, care needs, and preferences. Candidates will review consumer medical documentation relevant to the cases. Data must be entered into electronic evaluation forms and transmitted as necessary. Documenting concerns and conflicting information during the evaluation process is essential. Emphasis on continuity of care is critical to reduce gaps in treatment plans. Candidates will discuss healthcare options and necessary supports with consumers. Communication with eligible consumers for enrollment into a managed care health plan will be initiated. Interdisciplinary collaboration to improve care coordination is necessary. Quality review data must be collected to support outcome measurements. Identification of opportunities for health promotion and illness prevention is important. A comprehensive working knowledge of community resources and payor requirements is required. Candidates must meet all established performance standards for the position. Other duties may be assigned by management as necessary.
