The typical work schedule runs from Monday to Friday, 8:00 AM to 4:30 PM, with the possibility of occasional evening or weekend hours based on client needs. Key Responsibilities\n \n Conduct detailed assessments of each senior's psychosocial needs, addressing emotional, mental, and social factors that affect their quality of life. \n Collaborate directly with seniors and their families to create personalized care plans that reflect their goals, preferences, and individual circumstances. \n Offer empathetic counseling and emotional support during critical life transitions, including health changes or loss of loved ones, assisting clients in navigating their challenges. \n Act as an advocate for client rights, ensuring they have access to vital community resources and support services. \n Engage and coordinate with interdisciplinary teams consisting of healthcare professionals, caregivers, and other stakeholders to streamline holistic care. \n Perform regular check-ins with clients to monitor their mental and emotional status, ensuring safety and addressing any emerging concerns. \n Facilitate effective communication between family members involved in the care of seniors, managing conflicts and fostering understanding. \n Educate clients and families on available support networks, community resources, and programs to encourage social engagement. \n Maintain meticulous documentation of client interactions, assessments, and care improvements in accordance with ethical standards and regulations. \n Patient Population / Client Focus\n The primary focus of this role is on seniors, many of whom face complex challenges as they navigate aging-related issues.
