Case Manager
Southwest Care Center
Southwest Care Center is seeking a dedicated Case Manager to collaborate with multidisciplinary teams in our Santa Fe and Albuquerque clinics. You will handle intake, assessments, care coordination, and connect clients to services to improve treatment adherence. You will document progress in the EMR, assist with insurance eligibility, and provide counseling on financial planning, housing, and access to medical and behavioral health services. Outreach HIV/HCV rapid testing may be part of the role. #J-18808-Ljbffr
