Nurse Practitioner – Value-Based Managed Care Location : Phoenix, Arizona Schedule : Full-time | PRN/Per Diem also available Compensation : Base Salary of $125,000 + Incentive (PRN/per diem: hourly/per-visit rate) About Akos MD Akos MD is expanding its Value-Based Care (VBC) at Home clinical team across the state of Arizona and hiring Family Nurse Practitioners ( FNP/AGNP/ACNP/PMHNP) to deliver outcomes-driven primary and preventive care within our value-based managed care service line. Most encounters are in-home visits, supplemented by virtual / telehealth visits. You'll complete health risk assessments, close care gaps, and connect Medicaid/Medicare members to the right level of care - backed by a support team of care coordinators. About the Role Akos MD is hiring a Nurse Practitioner to deliver outcomes-driven primary and preventive care within our value-based managed care service line. The majority of patient encounters in this role take place as in-home visits , with virtual (video/phone) visits used to supplement and extend access. You'll work with Medicaid and Medicare members to complete comprehensive health risk assessments, close preventive and chronic care gaps, and connect patients to the right level of care - backed by a full support team of care coordinators. This role sits at the center of our highest-growth service line, where clinical quality directly drives contract performance and enhanced payer incentive rates. What You'll Do Deliver primarily in-home primary care visits, Health Risk Assessments (HRAs), and preventive/wellness visits for Medicaid and Medicare managed care members, including hard-to-reach and previously disengaged patients Serve as Primary Care Provider (PCP) of record for an assigned panel, managing chronic and complex conditions and developing individualized care plans Provide care within scope of practice and in accordance with a collaborative practice agreement/supervising physician arrangement, per state licensure requirements Order, interpret, and act on diagnostics; prescribe medications within DEA and state scope-of-practice rules Provide timely follow-up consistent with access-to-care standards Complete comprehensive assessments covering medical, behavioral, and social determinants of health, with rapid routing to appropriate care and community resources Quality, Coding & Compliance Accurately document to support risk-adjustment coding and reflect true member acuity Identify and close preventive and chronic-care quality gaps in support of payer quality/performance measures Maintain complete, accurate medical records per state and federal recordkeeping and retention requirements Complete required credentialing and recredentialing, and maintain good standing throughout employment (active license, no exclusions from federal/state healthcare programs, required insurance coverage) Comply with Akos MD's and payer partners' clinical policies, utilization management, referral/prior-authorization procedures, and coding/documentation standards Participate in required compliance training (fraud, waste & abuse; HIPAA; documentation integrity) Exercise independent clinical judgment in all treatment decisions Care Coordination & Technology Collaborate with Akos MD's care coordination team and physician leadership as part of a multi-channel member engagement model Ensure timely notification/escalation for hospital admissions, ED visits, missed appointments, or significant changes in a member's condition, consistent with care coordination protocols Use AI-assisted documentation and clinical workflow tools within Akos MD's cloud-based platform to reduce administrative burden Support timely, accurate data entry into the EHR to enable continuity of care and required reporting What We're Looking For Active, unrestricted Nurse Practitioner license in the State of Arizona; certified NP licensure recognized as a PCP-eligible provider type under Arizona Medicaid (AHCCCS) rules National board certification (FNP, AGNP, ANP, or related) Master's or Doctorate in Nursing from an accredited program Active DEA registration (or eligibility to obtain), as applicable to role scope Medicare and/or Medicaid participating provider status (or ability to enroll) Clean background check; no current or past exclusion, suspension, or debarment from federal or state healthcare programs Professional liability insurance meeting standard managed-care credentialing minimums (or ability to obtain) Bilingual fluency in English and Spanish preferred, to effectively communicate with and serve our diverse member population Experience with Medicaid and/or Medicare managed care populations strongly preferred Prior experience in value-based care, risk-adjustment documentation, or managed care quality programs (HEDIS, Star Ratings) preferred Comfortable delivering care in patients' homes as the primary care setting, with additional comfort using telehealth/virtual visit platforms Reliable transportation and willingness to travel to member homes across assigned service areas Strong communicator, comfortable engaging members who may be disengaged from the healthcare system, with cultural competency across diverse populations and, where applicable, limited-English-proficient members Self-directed and comfortable working within a distributed, technology-enabled care team Why Join Akos MD Join a physician-founded, physician-led company at an inflection point of rapid growth Be part of a model built to reward quality outcomes and gap closure Work alongside a dedicated care coordination team supporting every member you see AI-assisted clinical tools designed to reduce documentation time, not add to it Ground-floor opportunity as Akos MD expands its value-based care model to additional states and payers #J-18808-Ljbffr Akos MD
