Job Summary The Nurse Practitioner or Physician Assistant will provide comprehensive primary care services in an outpatient family medicine setting. This role includes delivering preventive, acute, and chronic care to a diverse patient population while supporting quality initiatives and coordinated, team‑based care. The provider will maintain accurate documentation and contribute to ongoing practice‑wide quality improvement efforts. Job Duties & Responsibilities Perform annual wellness exams and provide preventive, acute, and chronic care Manage a diverse panel of patients across all ages Maintain accurate and timely documentation in the EMR Collaborate with clinical staff to ensure coordinated, patient‑centered care Support managed care initiatives and quality programs Provide patient education to promote a healthy lifestyle and disease prevention Prerequisites / License & Certification Requirements Board Eligible or Board Certified Nurse Practitioner or Physician Assistant Valid Arizona medical license (or ability to obtain) Current DEA registration Experience in outpatient primary care preferred Strong patient communication and interpersonal skills If you meet all of our criteria and would like to be considered, please apply with your most updated Resume/CV.
