Registered Nurse - PACT Outpatient

Job Title

The duties of this position include but are not limited to:

• Provide competent, evidence-based outpatient nursing care to assigned patients.
• Oversee and collaborate with licensed vocational nurses and nursing assistants as appropriate to the outpatient setting.
• Coordinate care focused on patient transitions, including continuity of care across the patient care continuum.
• Deliver and support patient and family education, promoting self-management following discharge.
• Support customer satisfaction by addressing factors that influence the Veteran's experience.
• Assist in directing nursing education, orientation, competencies, and contribute to quality improvement and outcomes utilization.
• Direct other healthcare providers within scope, ensuring safe and effective care delivery.
• Partner with patients, families, and the interdisciplinary team to promote safe care transitions across the health care continuum.
• Apply fundamental outpatient nursing experience, demonstrating theoretical knowledge, critical thinking, and technical competency.
• Perform additional duties as assigned, consistent with PACT outpatient RN responsibilities.
• Support all three VA-NYHCS campuses, with rotation as needed.
