Oasis Therapy Services is seeking an Occupational Therapist (OT) to provide one‑on‑one rehabilitation services in an assisted living and memory care setting in Beaver Dam, Wisconsin. Available employment options: Part-Time EMR: Net Health Responsibilities Evaluate residents and develop individualized treatment plans. Provide therapy focused on ADLs, mobility, safety, and independence. Monitor progress and adjust treatment plans as needed. Educate residents and caregivers on exercises, adaptive techniques, and equipment. Complete accurate Net Health documentation and collaborate with the care team. Qualifications Active and current Wisconsin Occupational Therapist license. Graduate of an accredited Occupational Therapy program. Current CPR certification. Strong communication, organization, and documentation skills. Geriatric, assisted living, skilled nursing, or rehabilitation experience is preferred. Why Join Oasis?
