PTA Outpatient

We seek a highly skilled and experienced Registered Nurse with specialities in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, Operating Room. The Registered Nurse will be responsible for providing skilled nursing care, case management, and hospice care to individuals facing advancing age, frailty, and serious illness. Daily tasks include patient assessments, medication administration, treatment planning, and collaborating with the interdisciplinary team. Critical to this role is strong communication and strong collaboration with patients, providers, and other members of our team. At least 2 years of experience and exceptional skill is required. Responsibilities:

• Provide skilled nursing care including patient assessments and medication administration.
• Develop and implement treatment plans tailored to individual patient needs.
• Collaborate effectively with interdisciplinary team members to coordinate patient care.
• Manage case load including hospice care for patients facing advancing age, frailty, and serious illness.
• Maintain accurate and timely documentation of patient care and treatment progress.
• Communicate clearly with patients, families, and healthcare providers regarding care plans and patient status.

• Active Registered Nurse license in the state of employment.
• Bachelor's degree in Nursing or related field.
• Minimum of 2 years experience in hospital, home health, or hospice care.
• Clinical assessment, medication administration, and treatment planning skills.
• Strong interdisciplinary team collaboration abilities.
• Excellent communication and interpersonal skills.
• Driver's license, car insurance, and automobile for field work.

• Experience in specialized nursing areas such as ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room.
• Additional certifications related to hospice or case management nursing.

• Ability to work effectively in a fast-paced and dynamic healthcare environment.
• May require occasional weekend shifts or on-call availability depending on patient needs.

• Standard Monday through Friday schedule with occasional weekend shifts as needed.
• Shift assignments may vary based on patient care requirements and facility needs.
