St. Raphaels Home Care is a nurse-owned, rapidly growing Catholic home care company based in North Branch, Minnesota. We are dedicated to providing high-quality, compassionate care to our clientsprimarily veterans.

Position Overview

The Home Health RN is responsible for managing their schedule and ensuring timely completion of client visits. This role focuses on delivering skilled nursing care in patients homes while maintaining accurate and thorough documentation.

Typical responsibilities include:

• Medication setup

• Wound care

• Catheter care

• Other skilled nursing services

Training is provided for any unfamiliar skills. New graduates are welcome to apply.

Key Responsibilities

• Conduct in-home assessments for veterans

• Schedule and manage client visits efficiently

• Deliver skilled nursing care in the home setting

• Complete accurate and timely documentation

Qualifications

• Active Registered Nurse (RN) license (Required)

• Home care experience (Preferred, not required)

• Strong organizational and time management skills

• Attention to detail and willingness to learn

• Ability to work independently

Additional Requirements

• Ability to reliably commute or relocate

• Employer-provided relocation package available

• Work performed in-person in patient homes
