Service Coordinator (KRCC Southern Counties – Perry, Knott, Letcher, Leslie) Hazard or Emmalena or Mayking or Hyden, KY Company/Location: Kentucky River Community Care/Perry, Knott, Letcher, Leslie Counties Employment type: Regular Full-Time Credentials Required: Bachelor’s degree in an approved Human Services field (Social Work, Psychology, Sociology, etc.) with one year post‑graduate work with children. Special Skills/knowledge: Good communication skills, ability to maintain client‑related goal data including case notes, outreach, and relationships with community partners. Primary Functions Coordinate, link, monitor, and assist clients and families on caseload to identified services and needs. Collaborate with community partners and service team members. Work among and facilitate a service team. Identify and link clients and families to community resources. Maintain knowledge of community resources and how to access resources. Complete documentation and medical record requirements.
