Program CoordinatorExemptSalary: $50,000Sign-on bonus of $1,000 payable in two installments ($500.00 after the first 3 months, $500.00 after satisfactory completion of 12-month period.Job Purpose:Under the supervision of the Program Manager the Program Coordinator will have responsibility for carrying out the day-to-day functions of their assigned home. This includes securing the well-being and safety of individuals with disabilities which requires; assisting individuals with personal care and daily living skills; training and supervision of Direct Support staff; maintaining house/site facilities; and a broad range of program wide administrative tasks within designated area.This position requires a commitment to individual choice and community integration and inclusion. Travel by car is necessary and a great deal of time will be spent in individual houses/sites. This position requires the ability to work where needed within the Tri-County area. In addition, responding to emergencies during and after hours, on weekends, and holidays is required when applicable.Essential Duties and Responsibilities:Supervising Direct Support Professional (DSP)Schedule planning and reviewMaintaining adequate staffing ratiosApprove leave/timecard review and submit to Program Manager for reviewFill in for staff when needed to cover the shiftsAssist the Program Manager with interviewing and hiring of potential direct support staffProvides individualized orientation to new staff members, mentors and provides appropriate/constructive feedback, when applicableAssures adherence to policies and procedures through training feedbackDelegating and assigning duties to DSPsEnsuring review/signatures of all plans (nursing, behavior, etc.)Monitoring electronic health record for case notes and documentation (client dashboard)All assigned tasks are completed daily and logged accuratelyComplete house inspections/fire drills and monthly attendanceCoordinate/facilitate house meetingsFinances/ManagementEnsures that homes operate appropriately and within budgetary guidelines as well as maintains the highest possible quality of living standardsEnsures that in house consumer monthly finances (cash on hand) is managed appropriatelyMonitoring to not exceed given amount for benefits (spending as needed when determined by manager)Responsible for maintaining appropriate budgetary responsibility for Impress account (food) and SNAP (individual food account) DocumentationComplete the PCP in the electronic health record for reviewTraining staff on all PlansReview all incident reports for accuracy and corrections before submitting for reviewHealth and WellnessResponsible for all medical appointments/paperworkPrimarily responsible for tracking, carrying out and documenting all medical appointments for the individuals, delegating to DSP as neededSchedule and plan activities for consumers to participate in weekly/monthlyDuring emergencies, Manager for coverage while assisting at the hospitalMonitors routine and emergency medical/dental services and medication reviews as appropriateEnsures that DSPs monitors (to ensure proper nutritional value) and/or consumer hygiene practicesProfessionalismModels initiative and creative problem solvingDemonstrates appropriate workplace behavior, modeling the agency's core philosophyDemonstrates role model behaviorMaintains confidentiality in relation to individuals and staff issuesOther Duties & ResponsibilitiesMay perform other duties as assignedEducation and Experience Requirements (Minimum)Possesses a High School Diploma or equivalency is required. However, an Associate Degree in human services or related field is preferred.Two years of progressively responsible experience working with people with developmental and intellectual disabilities or a combination of education and experience equivalent to this requirementMust be personable, energetic and have strong communication skills, both orally and in writing.Must have knowledge of community resources availableMust have strong interpersonal skillsWillingness to work anywhere within the Tri-County area (Calvert, Charles and St. Mary's counties) as needed.Customer service-orientedComputer skills (familiar with Microsoft Office applications)Has a good command of the English language both oral and written and demonstrates these skills.Possesses a valid driving license with a good driving record, and access to a safe vehicle (must pass agency sponsored inspection) for transporting people receiving services.Possesses the ability to pass the criminal background check mandated by the State of Maryland.Work EnvironmentWhile performing the duties of this job, the employee is generally not exposed to weather conditions.The noise level in the work environment is usually moderate.Americans with Disability Specifications (Physical Demands)The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
