FamAli LLC partners with Arizonas Division of Developmental Disabilities (DDD) to deliver high-quality therapy services. We support individuals with developmental disabilities in achieving greater mobility, independence, and participation in daily life. Key Responsibilities
- Conduct DDD-compliant PT evaluations (gross motor, mobility, strength, balance, equipment needs).
- Develop and implement Individualized Treatment Plans (ITPs) based on ISP goals.
- Provide direct PT services in home, community, or facility settings.
- Recommend and train on mobility aids, orthotics, and adaptive equipment.
- Participate in ISP meetings and collaborate with DDD Support Coordinators.
- Educate caregivers and support staff on therapeutic exercises and safety.
- Maintain documentation per AHCCCS and DDD standards.
- Monitor progress and adjust treatment plans as needed.
- Stay current with Arizona PT licensure and DDD policies.
- Doctorate or Masters in Physical Therapy
- Arizona PT license (required)
- Experience with developmental disabilities (preferred)
- Fingerprint Clearance Card, drivers license, reliable transportation
- Ability to meet DDD credentialing requirements
