Description

• Conduct assessments of patients in inpatient and outpatient environments, creating personalized treatment plans
• Implement physical therapy therapies with a focus on improving mobility, strength, balance, and overall function
• Engage in teamwork with nurses, physicians, case managers, and rehabilitation experts to enhance patient outcomes
• Guide patients and families in understanding therapeutic objectives, planning for discharge, and developing home exercise routines
• Maintain accurate documentation of evaluations, treatments, and patient progress, adhering to facility and regulatory standards
• Participate in interdisciplinary team rounds and conferences to deliver coordinated care

Education

• A Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited program

Certifications

• State licensure as a Physical Therapist
• CPR certification is preferred
• Specialized certifications (such as for neurological or geriatric care) are beneficial but not required

Skills

• Prior experience is highly recommended
