Requirements
- Current CA Physical Therapist License
- Basic Life Support (BLS) certification
- Driver's License
- Auto insurance
- Auto registration
- Tuberculosis (annual) test
- MMR vaccination
- Varicella vaccination
- Hepatitis B vaccination
- Tdap vaccination (valid for 10 years)
- Annual flu vaccination
- Drug screen clearance
- Physical

Roles & Responsibilities
- Evaluate and treat patients with physical impairments or disabilities.
- Develop individualized therapy plans based on patient needs and goals.
- Implement therapeutic exercises and treatments to enhance physical function.
- Monitor patient progress and adjust treatment plans accordingly.
- Educate patients and families on rehabilitation strategies and home exercises.
- Document patient interactions and progress in accordance with facility policies.

Compliance / Notes
- All requested time off must be disclosed at the time of submission.
- Time off requested after acceptance will not be approved.
