In-Home Health - Nurse Practitioner or Physician Assistant (Part Time) Oxford ME

Signify Health, part of CVS Health®, is hiring a Part‑Time Clinician ( NP or PA ) to provide in‑home health evaluations and bridge gaps in care for members across the United States. Responsibilities Conduct 45‑minute in‑home visits, including medication and medical history review, physical evaluation, and, when ordered by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Use the Signify Health mobile app and clinical workflow; no prescribing, lab ordering, or treatment alteration required. Build trusted relationships and connect patients to appropriate care resources. Perform flexible travel locally and within licensed states as necessary; conduct virtual visits as needed. Qualifications Active, unrestricted NP or PA license in the coverage area(s). Board certification (required); multi‑state licenses preferred.
