Department Summary

UNT Health Clinical Practice Group’s Center for Older Adults provides comprehensive care plans that help prevent and manage the chronic diseases and long-term illnesses that may accompany the aging process. Services include:

Comprehensive Geriatric Assessment - Cognitive and neuropsychological testing in Fort Worth - Primary care - Alzheimer’s and Dementia evaluation and treatment - Well care for older adults - Onsite care at several of Fort Worth’s continuing care retirement communities - Palliative care - Physical therapy - Chronic care management (CCM services) - Neurology consultations for brain health evaluation, memory loss, dementia and movement disorders - Advance Care Planning.

Position Overview

The Licensed Vocational Nurse (LVN) provides nursing care and clinical support for patients in a healthcare setting. The LVN assists in the delivery of patient care for individuals who are ill, injured, or seeking preventive health services. Working under the supervision of a registered nurse, physician, or other licensed healthcare provider, the LVN performs clinical procedures, monitors patient conditions, and supports the healthcare team in implementing treatment plans.

Minimum Qualifications

Graduation from an accredited school of nursing and one (1) year of related experience; or any equivalent combination of education and experience. A valid Texas Licensed Vocational Nurse (LVN) license and a current CPR (BLS) certification are required.

Knowledge, Skills and Abilities

• Administers medications, injections, immunizations, or intravenous fluids as directed by a licensed healthcare provider and document administration in patient records.
• Observes patients and report changes in condition, including adverse reactions to medications or treatments, to appropriate healthcare personnel.
• Measures and records patients' vital signs including height, weight, temperature, blood pressure, pulse, and respiration.
• Prepares patients for examinations or procedures and assists healthcare providers during clinical visits.
• Provides basic nursing care and treatments such as wound care, dressing changes, specimen collection, and other routine procedures.
• Assists with patient intake, including obtaining health histories and documenting information in electronic health records.
• Responds to patient questions and determines appropriate assistance or referral.
• Assembles, maintains, and uses medical equipment and supplies necessary for patient care.
• Maintains accurate, timely, and confidential documentation of patient care activities.
• Supports clinical operations by preparing examination rooms and ensuring a safe and clean patient care environment.
• Works collaboratively with healthcare providers and staff to support patient care, preventive services, and health education.

Preferred Qualifications

The preferred candidate will possess the following additional qualifications: Bilingual competency (English/Spanish). Previous experience preferably with a geriatric population including telephone communication, ambulatory care, familiarity with adult vaccine schedule, patient rights, and safety. Experience with Medicare and Managed Medicare insurance plans.

Required License/Registration/Certifications

The successful candidate will possess the following required qualifications: Graduation from an accredited school of nursing. A valid Texas nursing License and a current CPR (BLS) certificate are required.

Job Duties

• Measure and record patients' vital signs, perform procedures and administer medications, injections and vaccines as ordered by clinician in the electronic health record. Assembles, maintains, and properly uses equipment in patient care delivery.o be entered by department.
• Return patient calls, and performs telephone triage following standardized protocols and clinicians guidance.
• Gauges patients overall health status updating clinicians as indicated. Provides oversight and guidance to medical assistants in collaboration with clinic supervisor/manager.
• Obtain insurance authorizations for referrals, procedures, and medications. Follows clinical protocols for medications refills. Completes patient forms as necessary. 
• Follows all departmental and organizational policies and procedures with regard to patient safety, quality management, infection control, and human resources. Ensure appropriate legal and regulatory standards. 
• Deliver culturally competent care that respects each patient's individual beliefs.
• Participate in process and quality improvement initiatives that aim to ensure patient safety and efficient clinical workflows.
• Coordination of care with internal and external partners.

Physical Requirements

• Repeating motions that may include the wrists, hands and/or fingers.
• Sedentary work that primarily involves sitting/standing.
• Lifting and Moving objects up to 20 pounds.
• Communicating with others to exchange information.
