The salary range above represents this position's earning potential. The anticipated hiring range for this position will be $82,352- $105,591.61, based on the candidate's qualifications and experience.

Montgomery County is seeking a skilled Community Health Nurse II to join the Assessments, Evaluation & Review Services (AERS) team. This role provides clinical assessments and interventions for medically fragile, disabled, and vulnerable individualsboth adults and childrenliving in the community. The nurse plays a vital role in preventing institutionalization, promoting safety, and supporting independence through timely evaluations and crisis response.

Conduct nursing assessments for State and County programs:

• Community First Choice (CFC)
• Community Personal Assistance Services (CPAS)
• Community Options Waiver (COW)
• Supporting Older Adults With Resources Program (SOAR)
• Medical Adult Day Care Waiver

Perform specialized evaluations:

• STEPS (nursing home risk)
• PASRR (mental illness/intellectual disability and nursing facility placement)
• GES (emergency room psychiatric risk)
• Respond to urgent situations involving abuse, neglect, or exploitation of vulnerable adults
• Provide crisis intervention, clinical recommendations, and follow-up care planning

The selected candidate will be required to successfully complete a pre-placement background check, medical exam, and drug screening.

Education: Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by the Maryland Board of Examiners with a Bachelor's Degree in Nursing.

Experience: Two (2) years of full-time equivalent post-licensure experience as a Registered Nurse, one year of which must have been in the nursing area of Community Health. (Community Health Nursing includes working in schools (School Health Services), Faith-Based Medical Facilities, community clinics, government agencies, and non-profit organizations.)

License: Possession of a current license to practice as a Registered Nurse in the Maryland State, of Maryland issued by the Maryland Board of Examiners. If the license is temporary, a regular Maryland RN license must be obtained within ninety (90) days of appointment.

The State of Maryland accepts as valid the current RN licensure from a candidate's primary state of residence if that state has an interstate compact with Maryland. The states having this interstate compact are Arizona, Arkansas, Colorado, Delaware, Idaho, Iowa, Kentucky, Maine, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, North Carolina, North Dakota, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, and Wisconsin. (This equivalency only applies to candidates whose primary residence is not Maryland).

Any Registered Nurse (RN) applying for licensure and living in a member state of the Multistate Licensure Compact must take the NCLEX exam in their home state.

• Experience working with clients with Intellectual Disabilities and/or Mental Illness.
• Experience with Community-Based Assessments and Waiver Programs.
• Multilingual Proficiency in Amharic, Chinese, French, Korean, Spanish, or Vietnamese.
