Montgomery County is seeking a skilled Community Health Nurse II to join the Assessments, Evaluation & Review Services (AERS) team. This role provides clinical assessments and interventions for medically fragile, disabled, and vulnerable individualsboth adults and childrenliving in the community. The nurse plays a vital role in preventing institutionalization, promoting safety, and supporting independence through timely evaluations and crisis response.
Conduct nursing assessments for State and County programs:
- Community First Choice (CFC)
- Community Personal Assistance Services (CPAS)
- Community Options Waiver (COW)
- Supporting Older Adults With Resources Program (SOAR)
- Medical Adult Day Care Waiver
Perform specialized evaluations:
- STEPS (nursing home risk)
- PASRR (mental illness/intellectual disability and nursing facility placement)
- GES (emergency room psychiatric risk)
- Respond to urgent situations involving abuse, neglect, or exploitation of vulnerable adults
- Provide crisis intervention, clinical recommendations, and follow-up care planning
The selected candidate will be required to successfully complete a pre-placement background check, medical exam, and drug screening.
Education: Graduation from a National League for Nursing accredited college or university or an equivalent program as determined by the Maryland Board of Examiners with a Bachelor's Degree in Nursing.
Experience: Two (2) years of full-time equivalent post-licensure experience as a Registered Nurse, one year of which must have been in the nursing area of Community Health. (Community Health Nursing includes working in schools (School Health Services), Faith-Based Medical Facilities, community clinics, government agencies, and non-profit organizations.)
License: Possession of a current license to practice as a Registered Nurse in the Maryland State, of Maryland issued by the Maryland Board of Examiners. If the license is temporary, a regular Maryland RN license must be obtained within ninety (90) days of appointment.
The State of Maryland accepts as valid the current RN licensure from a candidate's primary state of residence if that state has an interstate compact with Maryland. The states having this interstate compact are Arizona, Arkansas, Colorado, Delaware, Idaho, Iowa, Kentucky, Maine, Mississippi, Missouri, Nebraska, New Hampshire, New Mexico, North Carolina, North Dakota, Rhode Island, South Carolina, South Dakota, Tennessee, Texas, Utah, Virginia, and Wisconsin. (This equivalency only applies to candidates whose primary residence is not Maryland).
Any Registered Nurse (RN) applying for licensure and living in a member state of the Multistate Licensure Compact must take the NCLEX exam in their home state.

The applications of those individuals meeting the minimum qualifications will be reviewed to determine the extent, relevancy of training, and experience in the following areas:
- Experience serving at risk seniors and Adults and children with disabilities in a multidisciplinary setting.
- Experience working in Nursing Facilities and/or assisted living facilities.
- Experience working in a home care setting and/or providing interventions in the client's home.
- Experience functioning independently in a community setting with clients of all ages.
- Experience working with clients with Intellectual Disabilities and/or Mental Illness.
- Experience with Community-Based Assessments and Waiver Programs.
- Multilingual Proficiency in Amharic, Chinese, French, Korean, Spanish, or Vietnamese.

Montgomery County Ethics Law : Except as provided by law or regulation, the County's "Public Employees" (which does not include employees of the Sheriff's office) are subject to the County's ethics law including the requirements to obtain advanced approval of any outside employment and the prohibitions on certain outside employment. Our generous leave package starts with non-MLS permanent and term employees earning 15 days of sick leave and 15 days of annual leave in their first year, as well as up to three additional personal days each calendar year. Positions in MLS earn 35 days of paid time off each year. In addition, we offer all employees 10 paid holidays. Our leave policies adhere to the Family Medical Leave Act of 1993, and we provide six weeks of paid parental leave for new parents.
All permanent and term positions are eligible for robust retirement plans. Public safety positions are eligible for a defined benefit pension plan in which they are vested after only five years. Employees in non-public safety positions can participate in retirement plans that contribute up to 12 percent of their annual salary. All employees can also participate in a supplemental retirement plan, the 457 Deferred Compensation Plan, to contribute even more to their retirement goals.
Montgomery County employees are eligible to participate in the Federal Public Service Loan Forgiveness Program. Participants who meet all requirements may qualify for forgiveness on the remaining balance on their Direct Loans after making 120 qualifying monthly payments under a qualifying repayment plan while working full-time in a qualifying public service position.
