About the Position

Job Title

Occupational Therapist – Home Health

Location

Portland, OR

Schedule / Job Type

Contract | Days (5x8 Hours, 8:00–16:30) | Onsite / Field-Based

Key Responsibilities

• Provide occupational therapy services to home health patients in the West Portland area and surrounding communities
• Conduct patient evaluations and deliver skilled therapeutic interventions in a 1:1 home setting, visiting up to 6 patients per day
• Complete Oasis assessments accurately and in accordance with home health standards
• Document patient care using EPIC Remote Client and Rover mobile application
• Manage a daily caseload while minimizing drive time across a service area with an occasional maximum radius of approximately 60 miles per day

Required Qualifications

• Active Oregon Occupational Therapist license
• Minimum of 1 year of skilled home health experience
• Oasis experience required
• Basic Cardiac Life Support (BCLS) certification
• Primary series COVID-19 vaccination required
• Reliable personal transportation required

Preferred Experience / Skills

• Familiarity with EPIC Remote Client for home health charting
• Experience meeting productivity point expectations in a home health environment (standard expectation of 30 points per day)
