Your efforts will help patients access the care they need while driving growth for our agency.***Key Responsibilities**** Develop and maintain relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other referral sources.* Educate healthcare professionals and community groups about home health and hospice services and benefits.* Serve as a knowledgeable, compassionate resource for patients and families during the referral and admission process.* Document outreach activities and maintain accurate records of interactions.* Ensure all messaging reflects our commitment to dignity, comfort, and quality care.* Collaborate with care coordination teams to support smooth transitions for patients.* Monitor community needs and identify opportunities to improve access to home health and hospice care.***Requirements**** Bachelor’s degree in Marketing, Business Administration, or related field preferred.* Minimum 3 years of healthcare marketing experience; **home health and/or** **hospice experience strongly preferred.*** Strong communication, relationship-building, and public relations skills.* Ability to work independently while maintaining a compassionate, patient-first approach.* Demonstrated organizational skills and ability to meet outreach goals.***Benefits**** Flexible schedules* Medical / Dental / Vision Insurance* Paid Time Off* 401(k) matching* Employee assistance program* FSA / HSA* Life insurance* Employee Referral ProgramThe employer for this position is stated in the job posting.
