This role involves coordinating patient care and optimizing resource utilization for acute and post-acute skilled patients within a multidisciplinary environment.

Job Summary

As a vital member of the care team, you will assess the appropriateness and timeliness of care levels, ensuring efficient use of healthcare resources. Collaborating closely with interdisciplinary teams, you will support patient-centered, cost-effective care plans from admission through discharge, facilitating smooth transitions to home or other care settings.

Responsibilities

• Evaluate ongoing care needs and resource availability to assure appropriate utilization.
• Work daily with care teams to assess continued hospitalization and post-acute care requirements.
• Conduct patient rounds to perform assessments and follow-ups.
• Develop and implement goal-oriented, age-appropriate care plans.
• Coordinate care transitions and resolve discharge barriers.
• Educate patients and families on insurance guidelines and referral processes.
• Maintain thorough, timely documentation and assist with concurrent denial coordination.
• Adapt to changing environments and lead others through transitions.
• Build and sustain professional relationships fostering trust and positive experiences.
• Engage in ongoing professional development and team activities.
• Protect patient privacy and uphold ethical standards.

Education and Experience

• Minimum of 2 years Registered Nursing experience in acute care or care management/utilization review.
• Current RN license valid in the region or multistate.
• Bachelor's degree in Nursing preferred.
• Care Manager Certification is a plus.
