Direct Support Professionals (DSP) Needed - Brooklyn, NY

Part-Time Position | OPWDD / ID-DD Services

Bond Health is seeking a compassionate and dependable Direct Support Professional (DSP) to work with individuals with intellectual and developmental disabilities.

Schedule
• Tuesday: 12:30 PM - 8:00 PM
• Wednesday: 12:30 PM - 3:30 PM
• Thursday: 12:00 PM - 8:00 PM
• Saturday: 12:00 PM - 8:00 PM

Essential Functions and Responsibilities
• Assist individuals receiving services in leading self-directed lives and participating fully in their local communities.
• Support the emotional, physical, and personal well-being of individuals while remaining attentive and energetic in reducing risk of harm. Monitor each individual's ability to self-preserve and respond to harm or danger.
• Respect confidentiality and privacy in accordance with state and agency regulations, recognizing that confidentiality must be broken if there is imminent harm to the individual or others.
• Promote and ensure the human and civil rights of individuals receiving services and assist them in understanding and expressing their rights and responsibilities.
• Promote advocacy, inclusion, and full community participation. Communicate and teach in ways that are age- and developmentally appropriate.
• Ensure services and supports are delivered in accordance with agency policies and procedures and all OPWDD, State, and Federal regulations and guidelines.
• Identify, intervene, and report suspected neglect or abuse and, if requested, participate fully in investigations involving suspected or reported incidents.
• Be knowledgeable of each individual's Habilitation Plan, valued outcomes, and personal goals.
• Implement each individual's Habilitation Plan as written, document required data in the agency's electronic medical record system, and assess and monitor progress.
• Provide a therapeutic and professional environment and maintain professional behavior, conduct, and presentation at all times.
• As directed by the Program Supervisor, maintain appropriate communication with family members, service coordinators, and other pertinent members of the individual's life.
• Provide the appropriate level of support and supervision while actively engaging individuals in the program, community, and/or volunteer site.
• As needed, supervise, support, and/or assist individuals with activities of daily living, including eating, toileting, dressing, personal hygiene, and grooming.
• Participate in all scheduled or required meetings and trainings requested by the Agency or Supervisor.
• As requested, travel with individuals to and from agency-approved activities using public transportation.
• Perform other duties as assigned by the supervisor.

Qualifications
• High School Diploma and experience working with individuals with intellectual and/or developmental disabilities (ID/DD).
• Excellent written and oral communication skills.
• Flexibility in scheduled work hours as needed.
• Eager to learn and develop professional skills in the field.
• Ability to work with a diverse group of people and be a team player.
