Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This opportunity is ideal for a licensed pharmacist specializing in Medication Therapy Management (MTM) within a specialty MTM environment. The position offers a 13-week contract with the potential for direct hire, supporting full-time MTM pharmacist needs either onsite or remotely. Candidates must hold a current Louisiana pharmacist license along with a minimum of five additional state licenses from a specified list, with an emphasis on Louisiana, DC, North Dakota, New Hampshire, New Jersey, South Carolina, South Dakota, Utah, and Wyoming.

Key Responsibilities:

• Prepare and dispense drug orders accurately in accordance with physician requests and established protocols.
• Interpret both verbal and written prescriptions, ensuring accurate transcription to computerized medication profiles.
• Maintain complete, accurate pharmacy patient medication profiles.
• Dispense pharmaceuticals and controlled substances following legal requirements and internal policies.
• Monitor drug therapy regimens for potential contraindications, interactions, allergies, and dosage appropriateness.
• Detect and report adverse drug reactions promptly and accurately.
• Provide clinical consultation to practitioners, recommending cost-effective therapeutic alternatives.
• Document all clinical activities, interventions, and medication use reviews meticulously.
• Contribute to quality improvement initiatives and participate in department inspections and record keeping.
• Supervise pharmacy support personnel and verify their daily activities.
• Manage timely pharmacy services independently, prioritizing assignments effectively.
• Perform medication storage inspections, identifying and removing expired or unusable drugs.
• Professionally handle telephone and window requests.

Desired Qualifications:

• Current Louisiana pharmacist license required.
• Licenses in at least five additional states from the required list, with a goal of obtaining ten states within 18 months.
• Prior retail or hospital pharmacy experience.
• Experience in Medication Therapy Management, especially within a specialty MTM setting.
