Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.

This full-time Medication Therapy Management (MTM) Pharmacist position offers a 13-week contract-to-hire opportunity focused in a specialty MTM environment. The role is flexible, supporting onsite or remote work. Candidates must hold a current Louisiana pharmacist license and at least five additional licenses from key states, including DC, North Dakota, New Hampshire, New Jersey, South Carolina, South Dakota, Utah, and Wyoming. This position is distinct from short-term remote surge roles and is intended for sustained MTM pharmacist needs.

Key Responsibilities:

• Accurately prepare and dispense medications following physician requests and established protocols.
• Interpret both verbal and written prescriptions, transcribing them to computerized patient medication profiles.
• Maintain complete and accurate medication profiles for patients.
• Dispense pharmaceuticals and manage controlled substance distribution while adhering to legal requirements.
• Monitor drug therapies for contraindications, interactions, allergies, and dosage appropriateness.
• Identify and report adverse drug reactions promptly.
• Provide clinical consultations and recommend cost-effective therapeutic alternatives.
• Document all clinical interventions thoroughly.
• Participate in quality improvement initiatives and medication use reviews.
• Supervise pharmacy support staff and verify technician activities.
• Ensure timely pharmacy services through effective organization and prioritization.
• Conduct medication storage inspections, replacing outdated drugs as needed.
• Professionally manage communication via telephone and in-person requests.

Qualifications and Experience:

• Current pharmacist license in Louisiana plus multiple state licenses (minimum five from the specified list).
• Prior experience in retail or hospital pharmacy settings.
• Commitment to obtaining at least 10 additional state licenses within 18 months in this role.
• Strong knowledge of medication therapy management and pharmacy operations.
