SCAN Health Plan is seeking a Community Engagement Specialist to enhance frail seniors’ ability to age in place by delivering person-centered care and coaching. You will work telephonically and in-person to implement plans of care, advocate for members, and coordinate with healthcare and community resources. Responsibilities include conducting assessments, supporting Medi-Cal/Medicare dual-eligibles, and performing ECM activities across focus populations such as individuals experiencing #J-18808-Ljbffr SCAN Health Plan
